Now, your first appointment will probably last about 30 minutes. When visiting Western Sierra for the first time, please arrive at least 15 minutes before your appointment so we can get a copy of your insurance and ID. Also, we may need to confirm some information about your health history.
You can download several of the forms, print and complete them ahead of time. Drop off the forms or send them by U.S. mail to Western Sierra Medical Clinic at 844 Old Tunnel Road, Grass Valley, CA 95945, in Auburn at 3111 Professional Drive, Auburn, CA 95603 or 12183 Locksley Lane, Suite 106, Auburn, CA 95602, or in Kings Beach (Tahoe) at 8665 Salmon Ave., Kings Beach, CA 96143. You can also send them by facsimile to 530-273-7255. If you have questions on a form, do not sign it until you talk to a staff member. Once we receive your paperwork, you will be contacted by our Patient Intake Coordinator to set up an appointment or asked to provide more information.
If applicable, you should also bring your health insurance and/or Medi-Cal card along with a picture ID, and bottles of any medication you are currently taking. If this is a pediatrics visit, please bring your child’s immunization record to every visit.
